Leverage Your Nursing Skills to Help Drive Positive Outcomes in Innovative and Effective Model

This is an opportunity to apply your nursing knowledge and skills as part of a new and innovative program that is proving to be highly effective -- you will feel the impact of your efforts with patients every day. You will engage, enroll and coordinate care to improve health and quality of life for members of this program, helping them comply with treatment plans, take advantage of available resources, and more. All work is done over the phone, so you can work from the comfort of your home office and enjoy a good deal of autonomy. We're looking for sharp, creative professionals who are motivated to use their brainpower in a pioneering health management framework. While we are still young as a company, our successful approach has earned us contracts with major health plans and allowed us to expand rapidly. That growth will create opportunities for strong performers.

We have multiple opportunities for Care Coaches across the country, so feel free to share this information within your network.

Catasys is making a positive impact on people’s lives every day. We use predictive analytics to identify health plan members with unaddressed behavioral health conditions that worsen chronic disease, then engage, support and guide these members to better health with a personalized, human-centered approach. This has led us to where we are today: growing fast and saving lives as we do.

To support our explosive growth, we’re looking for compassionate, hard-working people-lovers to join our team. If innovating in the field of patient care is something you’re passionate about, we encourage you to join our mission to improve the health and save the lives of as many people as possible.

Their focus on the right care at the right time delivers the right outcomes for all involved.

The Role

Pivotal Role in Innovative Program

You will assume responsibility for engagement, enrollment, retention and coordination of care to improve the health and quality of life for our members. Key responsibilities will include:
  • Utilizing clinical and functional assessments of client progress, employing targeted intervention and education, and coordinating care pertaining to members’ identified issue(s).
  • Utilizing Motivational Interviewing techniques to elicit active member participation.
  • Coordinating member care throughout the treatment continuum, including medical intervention, relapse prevention, treatment of co-morbid conditions, and issues identified through the individual care plan.
Once you are up to speed with our model, you will work with an average of 60 to 80 members at any given time, depending on the types and scopes of the care plans. Typically your interaction with a member is more frequent early in the 52-week program and diminishes over time. However, you'll determine the necessary interaction level on a case-by-case basis.

Interaction will include both regularly scheduled phone meetings as well as ad hoc calls. Each case will be unique and require a different mix of intervention, motivation and care coordination. Care coordination will include interacting with healthcare providers, community resources, members' families, team members, and others.

More specifically, you will:
  • Use exceptional written and verbal communication skills to engage, enroll, and retain members in active participation in the  program as well as to coordinate member care.
  • Conduct superior motivational interviewing, enhancement, and relapse prevention guidelines in telephonic communication with members.
  • Demonstrate superior knowledge of the Catasys treatment program and usage of our electronic documentation platform.
  • Maintain a member-centric focus and provide exemplary customer service to all clients.
  • Communicate with or request information from providers in complete compliance with confidentiality standards and in a manner that generates effective communication and cooperation.
  • Comply with Catasys policies and procedures, including training, audits, alert response timeliness, intervention and follow-through, supervision receptiveness, and other Catasys related clinical/medical services.
  • Comply with Catasys human resource policies and procedures.
  • Support and meet any Catasys accreditation, auditing, and contracted stakeholder reporting expectation.
  • Work jointly with the Clinical Care Coordinator, Clinical Operations Coordinator, and Embedded Therapist to support member referral, engagement, and enrollment into the program.
  • Support enhancement of the electronic documentation platform by providing feedback to the development team and performing ad hoc system testing as indicated.
  • Adhere to Catasys' Standards of Behavior, which include compassion, excellence, integrity, professionalism, respect, and teamwork.
  • Understand and implement the Catasys Vision (Behavioral health no longer negatively impacts Physical health), Mission (Improve health and reduce cost of care for people who suffer from the medical consequences of behavioral health conditions, helping these people and their families maintain better lives) and Values (Member first, team powered, continuous improvement, dare to innovate, driven to outcomes).

Our model is research-based and we are invested in staying on the leading edge of treatment. You'll help us break down barriers and stigmas associated with mental health.

  • Practice conscientious work habits.
  • Remain adaptable, flexible, and able to manage multiple activities and priorities in a fast-paced, fluid environment.
  • Bring strong organizational skills and a detail orientation.
