The Requirements
To meet the basic qualifications for this role you will have legal authorization to work permanently in the United States for any employer without requiring a visa transfer or visa sponsorship. To be a good fit for the Care Coach opportunity, you also should have:

- 3+ years of direct health-care experience.
- Excellent written and verbal communication skills Demonstrated proficiency in Microsoft Office product.
- Experience with electronic medical record documentation.
- Health Coach Certification from a program approved by the National Board for Health & Wellness Coaches (NBHWC), or the ability and willingness to successfully receive such certification within 1 year of hire.
- A background in behavioral health is preferred.
- Experience with motivational Interviewing techniques is a plus.
A laptop, VoIP phone, headset and monthly stipend for Internet service will be provided. You'll need a home office space free from distraction, high-speed Internet connectivity that delivers download speeds of at least 10 MBPS and upload speeds of at least 5 MBPS, and the ability to hardwire the VoIP phone to your router.
Depending on need, you will be assigned to one of the following 8- or 10-hour shifts:
- 8-hour shifts
- Monday - Friday, 12 PM to 9 PM EST
- Tuesday - Saturday, 10 AM to 7 PM EST
- 10-hour shifts
- Tuesday - Friday, 10 AM to 9 PM EST
- Wednesday - Saturday, 8 AM to 7 PM EST

The Role
Pivotal role in innovative program
You will be responsible for engagement, enrollment, and coordination of care to improve the health and quality of life for members of the Catasys OnTrak program. This will include utilizing existing clinical and functional assessments of client progress, employing targeted intervention and education, and coordinating care pertaining to each client's identified issue(s). You'll also utilize motivational Interviewing techniques to elicit active member participation.

In addition, You'll be responsible for the coordination of member care throughout the treatment continuum: medical intervention, relapse prevention, treatment of co-morbid conditions, and issues identified through the individual care plan.
More specifically, you will:
- Engage, enroll, and maintain members in active participation in the OnTrak program as well as coordinating member care.
- Utilize motivational interviewing, enhancement, and relapse prevention guidelines in telephonic communication with clients.
- Use the Catasys OnTrak treatment program and eOnTrak electronic documentation platform.
- Provide exemplary customer service to, and member-centric focus for, all clients.
- Communicate or request information from providers in complete compliance with confidentiality standards and in a manner that generates cooperation.
- Work jointly with the Clinical Operations Coordinator and Community Care Coordinator to support member referral, engagement, and enrollment into the OnTrak program.
- Support enhancement of the digital tools (eOnTrak) by providing feedback to the development team and performing ad hoc system testing as indicated.
- Perform other duties as assigned.
